Hi, I am Rachel Pierre‑Chery, a Family Nurse Practitioner and the founder of Here For You Health Care, LLC. I created this practice because I truly believe that health care should feel personal, compassionate, and easy to access.
Over the years, I have had the privilege of caring for patients from different backgrounds and across all stages of life. My experience as a Family Nurse Practitioner has taught me that every patient has a story, and every patient deserves to be treated with kindness, respect, and dignity. I understand that life can be busy, and sometimes it is hard to make time for your own health — that is one of the reasons I wanted to offer telehealth services, to make care more convenient and supportive for individuals and families.
My goal is to listen to you, understand your concerns, and work with you to create a care plan that fits your needs. I believe in educating patients, answering questions, and helping you feel confident about your health decisions. Whether you need a follow‑up visit, medication management, chronic condition support, lab review, or guidance about your health, I am here to help.

What services do you provide?
Telehealth visits, primary care follow‑up, medication management, medication refills when clinically appropriate, chronic condition follow‑up, lab review, diabetes and blood pressure follow‑up, weight management support, health education, preventive counseling, and referrals when needed.
Are visits in person or online?
Most visits are provided through telehealth, meaning you meet with your provider by secure video or phone from a private location. If your concern requires an in‑person exam, urgent care, emergency care, labs, imaging, or a specialist, your provider will guide you on next steps.
Is telehealth safe and private?
Yes. We take patient privacy seriously and follow HIPAA guidelines to protect your health information. We encourage patients to attend telehealth visits in a private place and avoid public Wi‑Fi when possible.
Can I be seen if I am in another state?
Telehealth rules may depend on the state where you are physically located at the time of the visit. Please let the office know your location before each appointment so we can confirm whether care can be provided.
Preparing for your visit
What should I have ready for my telehealth visit?
Your photo ID, insurance card if applicable, medication list, allergy information, preferred pharmacy, recent blood pressure or blood sugar readings if available, and any questions you’d like to discuss. You may also be asked for your current physical location and an emergency contact at the start of the visit.
Do I need to complete forms before my first visit?
Yes. New patients may be asked to complete a new patient intake form, medical history form, medication list, allergy information, telehealth consent, HIPAA acknowledgment, financial responsibility agreement, and release of information if needed.
What happens if my video visit disconnects?
If the telehealth connection fails, the provider or office may call you using the phone number on file — please make sure your contact information is current before the visit.

Do you accept insurance?
We may accept insurance, cash pay, or both depending on the current practice setup. Please contact the office before your appointment to confirm accepted insurance plans and payment options.
What are your cash pay fees?
Fees may vary depending on the type and complexity of the visit:
- New Patient Telehealth Visit (30–45 min) — $150
- Established Patient Follow‑Up (15–30 min) — $100
- Medication Refill Visit (15–20 min) — $75
- Lab Review Visit (15–20 min) — $75

When is payment due?
Payment is due at the time of service unless other arrangements have been made in advance, and may be required before the telehealth visit begins.
What payment methods do you accept?
Credit card, debit card, HSA card, FSA card, or other approved payment methods.
Cancellations & no‑shows
What is your cancellation policy?
Please contact the office as soon as possible to cancel or reschedule. A $50 fee is charged for all no‑show appointments and same‑day cancellations. This fee may not be covered by insurance and may be your responsibility.
What is considered a no‑show?
A no‑show means you do not attend the scheduled appointment and do not notify the office in advance.
What is considered a same‑day cancellation?
A same‑day cancellation means you cancel the appointment on the same day it is scheduled.
Medications
Can I get medication refills through telehealth?
Refills may be provided when clinically appropriate. They are not guaranteed and may require an evaluation, updated medical history, vital signs, lab work, or an in‑person visit depending on the medication and your health condition.
How early should I request a medication refill?
At least 3–5 business days before you run out, to avoid delays and allow time for review.
Do you prescribe controlled medications?
Controlled medications, if prescribed, must follow federal and state regulations. Telehealth visits do not guarantee that controlled medications — or any medication — will be prescribed. Your provider will determine what is safe and appropriate based on your condition.
Safety & privacy
Can I use telehealth for an emergency?
No. Here For You Health Care, LLC does not provide emergency services. If you are having chest pain, severe shortness of breath, stroke symptoms, severe allergic reaction, severe bleeding, suicidal thoughts with plan or intent, thoughts of harming others, loss of consciousness, or severe confusion — call 911 or go to the nearest emergency room. For mental health crisis support, call or text 988.
Will my information be kept confidential?
Yes. We follow HIPAA privacy standards to protect your health information. Your information is only used or shared as allowed by law, for treatment, payment, or health care operations — or with your written permission when required.
What if I need in‑person care?
If your provider feels your condition cannot be safely treated through telehealth, you may be advised to see an in‑person provider, urgent care, emergency room, specialist, or complete labs or imaging.
Can you order labs or refer me to a specialist?
Yes, when clinically appropriate. Your provider will discuss the reason for the lab or referral and explain next steps.
How soon will I receive a response to messages?
For non‑urgent questions, refill requests, or appointment concerns, please allow 24–48 business hours. Do not use email, text, voicemail, or portal messages for emergencies.
